Submit a question or concern to Huntsville's Public Works Department. The submissions to this form are monitored during regular business hours, at which time will be transferred into a public service request ticket for you to receive status updates.

During a storm event of any sort, we experience a higher volume of inquiries therefore a written or phone reply to your submission may not be possible.

Personal information contained on this form is collected under the authority of the Municipal Act, 2001, c.25, as amended and will be used, maintained and disclosed in accordance with the Municipal Freedom of Information and Protection of Privacy Act c. M56 s.29(2) and will be used for the purpose of identifying and verifying the location of problem roads and sidewalks and to contact citizens about the issue. Details on this form will be disclosed to the appropriate staff contact(s) to assist in the information delivery and response. Information submitted by a citizen may be shared with officials, agencies and/or departments of the Corporation of the Town of Huntsville in assisting and addressing the concern/inquiry.
